Shipper's Declaration for Dangerous Goods
The shipper is responsible for the completion of this prescribed declaration form for each and every shipment containing dangerous goods.
For each shipment containing dangerous goods the shipper must:

  • Use the only correct form in the correct manner.
  • Complete the form accurately and legibly.
  • Ensure that the form is properly signed when the shipment is presented to the operator.
  • Ensure that the shipment has been prepared in accordance with these regulations.

This declaration form must be printed according to the following instructions:

Format: This form must be printed in the same format
Language: English.
Colour: May be printed in black and red on white paper or only in red on white paper.
Size: Standard ISO A3 : 297 x 420 mm ( 113/4 in .) x ( 161/2 in.)
Standard ISO A4 : 297 x 210 mm ( 113/4 in .) x (81/4 in.)

North American equivalents are :
Ledger 280 x 430 mm., 11 x 17 in.
Letter 280 x 215 mm., 11x 81/2 in.

Number of Copies:
Two copies of the declaration form must be completed and signed for presentation to the operator with the shipment.
